In Rochester in July, hospital management moved to cut health benefits for nonunion staff, and the union's nurses stood with those coworkers instead of letting the cut go through quietly. Many of those coworkers have since voted to join the union themselves. Where the federal ledger shows appropriated dollars sitting idle, the nurses' floor shows the opposite motion: workers closing a benefit gap without waiting for Washington to release the money.

Northwell's spike also sits inside cancer science that has been settled for over a decade. In 2015 the World Health Organization's cancer agency, after a review by 22 experts from 10 countries, classified processed meat as a known human carcinogen and red meat as a probable one. A CBS report published in July found lung cancer has overtaken breast cancer as the leading cancer killer of American women, with a growing share of cases among people who never smoked. The agencies that classify carcinogens and the doctors who screen for them have had the tools for years; what moves slower is the appropriation that pays for the screening.

Northwell's number came from a report. The underspending came from a report. The mayor's verb came from an op-ed page. Only one of the three has a dollar figure attached to fixing it, and it is not the one that ran under the mayor's name.
