On self-reliance

Ninety-one million. That's The Lancet's count of deaths the dismantled program prevented over twenty-one years 1, and I'll be honest, a number that size doesn't fit in the notebook, so I wrote it on the cover. Its absence has a projection — 9.4 million additional deaths by 2030 across all the donor cuts, and a projection is what it is 2 — but the built thing was real; I'm only reading the count of it.
What helps me with scale is the other people filling the gap: China's first entry into HIV assistance is a $3.49 million grant, to a country where American support ran $400 million a year 3. And the bill has a way of coming home in carry-on luggage — 2,777 measles cases this year, a 35-year high, the majority imported by unvaccinated Americans returning from abroad 4. I'll want to ask what the line item was called, the day it was cut. I'm not asking yet.

SELF-RELIANCE
The word in the doctrine is self-reliance, and I like a word with a definition, so I went looking for the paperwork behind it. Here's what it came with: 34 bilateral health agreements replacing the old programs, $24.3 billion over five years, the American share cut 34 percent against prior funding for the same countries, the partners now billed for 39 percent 1. And the State Department's own first-year results sheet — their sheet, not mine — counts 77,163 fewer children on HIV treatment than the year before, and 1,714 service sites closed 2.
Then a couple of loose pages I'll staple to the file: the polio eradication effort absorbing a 30 percent cut, with a $1.7 billion gap through 2029 3, and donor-government HIV funding worldwide at its lowest since 2007 4. I don't want to jump ahead. I'm just noting that the word came with a lot of subtraction in the attachments.
